Composition :

Cetirizine diHCl

Caplet 10 mg (film-coated) x 30’s

Drops 20 mL

Action / Pharmacology:

Cetirizine is an antihistamine with a low potential for drowsiness at pharmacologically-active doses and with additional antiallergic properties. It is a selective H1-antagonist with negligible effects on other receptors, and so is virtually free from anticholinergic and antiserotonin effects. Cetirizine inhibits the histamine-mediated ‘early’ phase of the allergic reaction, and also reduces the migration of inflammatory cells and t he release of mediators associated with the ’late’ allergic response.

Peak blood levels of 0.3 mcg/mL are reached between 30 and 60 min after administration of a 10-mg dose of cetirizine. Its plasma half-life is approximately 11 hours. Absorption is very consistent from one subject to the next. Its renal clearance is 30 mL/min and the excretion half-life is approximately 9 hrs.
Cetirizine is strongly bound to plasma proteins.

Indication :
Treatment of perennial rhinitis, seasonal allergic rhinitis and chronic idiopathic urticaria

Dosage & Administration:

Adults and Children >12 years: 10 mg daily.
At present, there are insufficient clinical data to recommend the use of cetirizine in children <12 years.
At present, there are no data which suggest that the dose needs to be reduced in elderly patients. In patients with renal insufficiency, dosage should b reduced to 5 mg daily.

Patients with a history of hypersensitivity to any of TIRIZ®’s constituents.
TIRIZ® is contraindicated in lactating women since the active ingredient, cetirizine, is excreted in breast milk.

Effects on the Ability to Drive or Operate Machinery: Studies in healthy volunteers at 20 and 25 mg/day have not revealed effects on alertness or reaction time; however, patients are advised not to exceed the recommended dose if driving or operating machinery.

Use in pregnancy

No adverse effects have been reported from animal studies. There has been little or no use of TIRIZ® in pregnancy. As with other drugs, the use of TIRIZ® in pregnancy should be avoided.

Side Effect:

There have been occasional reports of mild and transient side effects eg, headache, dizziness, drowsiness, agitation, dry mouth and gastrointestinal discomfort. If desired, the dose may be taken as 5 mg in the morning and 5 mg in the evening. In some individuals, hypersensitivity reactions, including skin reactions and angioedema may develop.

To date, there are no knwn interactions with other drugs. Studies with diazepam and cetirizine have revealed no evidence of interactions. As with other antihistamines, it is advisable to avoid excessive alcohol consumption.
